Self Care & Support

If you are living with a long term condition such as diabetes or asthma, monitoring your health and administering self care is an important requirement in ensuring you live a healthier and more productive life. Maintaining your health and well being through self care may seem like a challenge but our team is here to provide you with the support you need to take charge.

Our doctors and nurses can provide:

  • information on your long term condition;
  • advice on administering self care;
  • recommendations on important lifestyle changes; and
  • supporting resources.

Support Groups


  • Diabetes clinic (Fridays 10-11am) Dr Love and Clinical Pharmacist Harry Ofori
  • Diabetes support group: meets 2nd Tuesday of the month (ask reception/email for details)
  • Youth-PPG: for all patients aged 13-18, meets every 2 months, education topics discussed have included drug and alcohol abuse, stress and exam pressures, what makes a good GP surgery
  • Substance misuse Lifeline shared care clinic: Tuesdays all day
  • Chronic pain clinic (Dr Love): Tuesday 10-12, Dr Love to assess any interested patients and book into this clinic (not directly bookable)
  • Gardening project: open to all patients, all ages. Meet Fridays 2-4pm
